Welcome to our practice! We are delighted that you have chosen to join our family of patients.
When you schedule your first visit with us, the necessary forms will be sent to you electronically to complete prior to your appointment. If you are unable to complete the forms electronically, the necessary forms are available below for you to print out, sign and bring with you to your appointment. You may also print and keep our HIPAA Privacy Policy form for your records.

When you arrive for your visit, you’ll immediately be greeted by our friendly staff who are available to assist with any questions you may have. You’ll be relaxed during your treatment in our beautiful, atrium-style operatories. We are pleased to be of service to you, and we know you’ll be delighted with the service you receive.
Your first appointment will be spent conducting a thorough dental examination including dental and medical history review, a visual exam of the oral tissues and teeth and a complete mouth X-ray survey. This comprehensive evaluation will allow us to thoroughly diagnose and provide your dental care.
In order for us to provide a thorough examination, it is important that we have recent X-rays. If you’ve had check-up X-rays taken within the past two years and/or a full set of X-rays taken within the past 5 years at another office, please contact them to request a duplication of these records. If the xrays are in digital form, they may be emailed to us at [email protected].
